PATIENT’S RIGHTS AND RESPONSIBILITIES

IT IS YOUR RIGHT AS NORDIC HOME HEALTHCARE PATIENT:

  1. To be informed (Patient & families) about the type of services NORDIC HHC provides and how to access those services.
  2. To be treated with dignity and respect, consistent with professional standards for all patients regardless of manner of payment, race, sex,
    nationality, religion, culture, disability or any other factors.
  3. To have considerate and respectful care consistent with patient’s personal values and beliefs.
  4. To receive emergency medical care and treatment quickly in an emergency situation regardless of the status of insurance card
  5. To make a decision about your treatment choices
  6. To give general consent for treatment and have the general scope and limits of this consent explained.
  7. To have informed consent explained in an understandable way to have adequate understandable information for you and your family about your
    illness, proposed treatment and care provided in order to make informed decisions.
  8. To be referred to a specialist/consultant for special care when there is a clinical need.
  9. To received detailed explanation of your condition, care treatment, and after care, in terms that is free from professional jargon in order that you
    can fully understand
  10. To be provided with Arabic/English interpreter services; and all other language barriers shall be accommodated based upon availability of
    translation resources.
  11. To have a second opinion or to have your care transferred to another physician if you are not satisfied with the patient care provided.
  12. To have all clinical records kept fully updated, relevant information fully documented and personal details and records kept fully confidential and
    protects from loss and misuse.
  13. To have any complaint that you make acknowledge, fully investigated, and be provided with a written response as per the facility policy
  14. To have privacy during examination, procedures, clinical care treatment and you have the right to know who is in attendance and the purpose of
    those in attendance on them.
  15. To be informed regarding treatment charges any uncovered costs and expenses prior to making decisions.
  16. To receive appropriate protection if you are elderly, vulnerable and disabled children.
  17. To be protected from physical assault during your visit in NORDIC HHC.
  18. To be able to identify visiting personnel members through proper identification
  19. To be informed of any financial benefits when referred to NORDIC HHC.

IT IS YOUR RESPONSIBILITY AS NORDIC HOME HEALTH CARE PATIENT:

  1. To bring your medical card/insurance card with you every time you attend NORDIC HHC.
  2. To follow the rules and regulations of NORDIC HHC
  3. To provide accurate information about personal details, medical history medication you are receiving and history of allergies od sensitivity to
    medications.
  4. To participate to the best of your ability in making decisions about your medical treatment, to follow instructions and comply with the agreed
    upon plan of care.
  5. To ask questions to your physician or other care provider if you are not clear about any aspects of your health care provided.
  6. To consider the rights and responsibilities of other patients and health care professionals.
  7. To show consideration for other patient needs, especially where their needs are greater.
  8. To show respects and to be courteous to the staff
  9. To not use abusive language and / or display unsocial behavior to other patients, visitors or NORDIC HHC staff
  10. To be accountable for your own actions if you decided not to follow NORDIC HHC instructions and/or treatment plan recommendations.
  11. To safeguard any belonging that you decide to keep in your possession while receiving any treatment within NORDIC HHC
  12. To keep appointment and inform staff if you are unable to attend within 24hours, so that the appointment may be used for someone else in
    need.
  13. To be accountable for the payment of the services provided by NORDIC HHC as applicable
  14. To be fully informed of one’s responsibilities.
